Application Instructions
Childhood Cancer Survivor Scholarship
Required Documents
- Completed application form (3 pages)
- Statement from your oncologist or current physician confirming that you were treated (or are currently receiving treatment) for childhood cancer (age 0-18 years)
- 300-500 word letter describing your personal life experience, your future goals, and why you are applying for this scholarship. We want to get to know you! Please include any details that you feel are relevant
- Letter of acceptance from the post-secondary institution you will be attending (or are currently attending)
- A current digital photo (jpeg) of yourself (please only include solo shots)
- Confirmation of enrolment in your post-secondary program and proof of fees paid will be required once school has begun.
Application Process
- Only PDF files will be accepted
- Please scan each document as a separate file and email to gillian@childhoodcancer.ca (documents that consist of multiple pages to be saved as one PDF. Example, your 3 page application would be sent as 1 pdf)
- Mark subject line of email as ‘2012 Scholarship – Your Name’
- All applications are due no later than Friday, July 13, 2012 at 5pm
- Late applications may be considered for the following year
- Confirmation that your application has been received and will be sent at the end of the week
- All applicants will be notified by email about status of scholarship.

TEVA Canada Scholarship

Required Documents
- Completed application form. (3 pages)
- Personal essay (4 pages typed max) describing how your journey with cancer inspired you to follow a career in the field of medicine/pharmacy/health sciences. Please also address how the TEVA Canada Scholarship will help you financially in your academic career.
- Copy of your undergraduate degree.
- Letter of acceptance from the univeristy you will be attending for post-graduate program.
- Statement from your oncologist or current physician confirming that you were treated (or are currently receiving treatment) for childhood cancer (age 0-18 years)
- A current digital photo (jpeg) of yourself (please only include solo shots)
- Confirmation of enrolment in your University program and proof of fees paid will be required once school has begun

Selection Process
- Applications undergo initial review to ensure that all the information requested has been submitted and that the required criteria has been met.
- Each qualified applicant will be reviewed by the TEVA Canada Scholarship Selection Committee in summer 2012
- Telephone interview will be scheduled for each finalist
- Scholarship recipients will be selected on the basis of criteria met, interview, and funds available for 2012
- TEVA Canada Scholarship winners will be notified by telephone
- Unsuccessful applicants will be notified by email but are encouraged to apply again next year
- TEVA Canada Scholarship cheques till be mailed via courier in Semptember/October 2012 to the address listed on your application (please notify us before this time if there is a change)
After your application is submitted
- Confirmation that your application has been received will be sent at the end of the week that it was received in
