If you are selected to advance to the next stage of the application process, you may be asked for additional supporting documentation. We will evaluate your initial application and will contact you as needed based on the information you provide below.

In order to receive your scholarship cheque you must provide your SIN #, as a T4 will be issued for tax purposes.

Please also include any challenges you have faced financially and how this scholarship will impact you and your family. Do not include experiences or skills that aren’t relevant to the application.

This authorization is continuous and may only be withdrawn by my specific rescinding of this authorization. Consequently, Childhood Cancer Canada may publish materials, use my name (first name only), photograph, and/or make reference to me in any manner that the Foundation deems appropriate in order to promote/publicize service opportunities.
